Please arrive 15 minutes prior to your scheduled appointment to fill out intake forms. Alternatively, you may complete these forms (see below) prior to your appointment.
Co-pays are due at the time of service.
If you are transferring care from another provider, please have your records faxed to our office at 815.941.5165.
Minors must be accompanied by their legal guardian for their first appointment.

Insurance Accepted
We accept most major health insurance plans; however, it is your responsibility to contact your insurance carrier to verify coverage and benefits. Patients may be held financially responsible for payment of services should the insurance company deny responsibility. Be sure to bring your insurance card with you every time you visit Morris Dermatology/Joliet Dermatology. We file secondary claims as a courtesy for our patients.
Please make certain to obtain referrals if your plan calls for it and in certain cases, know that plans require prior authorizations for procedures. Please note that Morris Dermatology/Joliet Dermatology does not participate in Medicaid programs or Independent Physician Associations. Our billing office can assist with obtaining a prior authorization and verifying network status if needed. Please call 866-701-4710 with questions regarding insurance.

Cancellation Policy
If you are unable to keep your appointment, please notify us at least 24 hours in advance.
Prescription Refills
If you require a medication refill, please have your pharmacy fax a refill request to 815.941.5165. You may also call our office to request a refill. Requests should be discussed with the nurse. Please note that no refills will be given if the patient has not been seen by Dr. Jenkins within the last year.
