To schedule an appointment, please call (815) 941-5160.

Office Hours

Monday - Thursday: 9 am to 5 pm
Friday: 9 am to 3 pm

New Patients

You must provide a photo ID and insurance card at your appointment. We also require the social security number and birth date of the guarantor of the insurance policy for timely filing.

Please arrive 15 minutes prior to your scheduled appointment to fill out intake forms. Alternatively, you may complete these forms (see below) prior to your appointment.

Co-pays are due at the time of service.

If you are transferring care from another provider, please have your records faxed to our office at
(815) 941-5165.

Minors must be accompanied by their legal guardian for their first appointment.

Returning Patients

You must provide a photo ID and insurance card at your appointment. We also require the social security number and birth date of the guarantor of the insurance policy.

For your convenience, you may print and complete these forms. Please bring them to your first appointment.

Payment Methods

We accept payments by cash, check and credit card (Visa, Mastercard, American Express, Discover).

Billing Inquiries

Cancellation Policy

If you are unable to keep your appointment, please notify us at least 24 hours in advance.

Prescription refills

If you require a medication refill, please have your pharmacy fax a refill request to (815) 941-5165. You may also call our office to request a refill. Requests should be discussed with the nurse. Please note that no refills will be given if the patient has not been seen by Dr. Jenkins within the last year.
